If you think that you may be a candidate for anesthesia-based chronic pain management services, please discuss your condition with your primary care physician to determine whether you qualify for a referral.  Also, please check with your insurance company to determine coverage prior to your appointment.

The clinic is open Monday through Friday, 8:00 a.m. to 4:30 p.m.  Please be ready to provide detailed information regarding your condition as well as the name and location of your referring physician.
